Does anyone know what frequency(s), modulation, and power output the CM and LEM used?
How many different radios (main/backup) did they have?

I don't have the information at my fingertips, but I might be able to find it.

I believe the frequencies used were around 2295 Mhz, though I don't know the exact number. Much of NASA's communications are around this frequency. In the air-ground communications you'll often heard them refer to it simply as UHF.

I don't have the immediate answer, but some of this was covered in the podcast series "Conversations with Apollo".
