I a movie about the SMART-1 mission, I saw a presentation by NASA administartor O'Keefe from the time the Prometheus initiative (nuclear-electric propulsion for interplanetary probes) was put forward.
He talked about, that it usually takes 15 years for a probe to reach its target (don't know about which he talked) and you've only one chance at the fly-by. Then he said, "And then, maybe the camera is broken or the film is bad".

IIRC, the last probes that went to other heavenly bodies equipped with film were some Luna probes and the Lunar Orbiters. And not even then was the film returned, it was just scanned.
